Muenster Family Medical Clinic will be conducting sports physicals at school on Tuesday, May 29, 2012. We will begin at 3:00 p.m. Payment is due on the day physicals are performed. The cost is $15 per person. Checks should be made payable to: Muenster Family Medical Clinic. All money collected for the physicals will be returned in the form of a donation to the school’s booster club. Students must have the Medical History side of the Evaluation Form filled out and signed by the parent before we can perform the physical.
Again this year, we will be offering the service of conducting FREE EKGs for students and sending them to a Pediatric Cardiologist to review. If any abnormal readings are found, parents will be notified. This offer is limited to the month of June and an advance appointment will be necessary.
